RESUMO

Collagenomas, or connective tissue nevi of the collagen type, represent a hamartomatous overgrowth of normal collagen. Isolated collagenoma is rare and most commonly presents in childhood. We herein report a rare case of congenital isolated collagenoma on the scalp without any associated abnormality. A 4-year-old boy with normal development presented with 2x1.5 cm sized, soft, dome-shaped brownish nodule on the scalp. Histopathologic study revealed collagen bundles with thickened appearance, a condition consistent with collagenoma.

RESUMO

Seborrheic keratosis is a common benign skin tumor originating from keratinocytes. In most instances, their etiology is unknown; however, there are independent risk factors such as genetic propensity, cumulative exposure to sunlight, human papillomavirus and epidermal growth factors. We report a case of seborrheic keratosis which occurred at the operation site 2 months postoperatively. This case may suggest that growth factors being induced by wound healing make a seborrheic keratosis on the operation site. To the best of our knowledge, this is a rare case of seborrheic keratosis on the operation site.

RESUMO

Milium-like syringoma is a rare clinical variant of syringoma, characterized by 1~2 mm, yellowish-white, domed papules. Milium-like syringoma can be clinically mistaken for milia because the lesions are usually similar in appearance. Histopathological examination showed keratin-filled cysts in the papillary dermis and numerous tadpole-shaped epithelial strands in the upper half of the dermis below the cyst. Moreover, most cases report the direct connection between the epithelial strands and the wall of the keratin-filled cyst. We report a rare case of milium-like syringoma in an 8-year-old girl.

BACKGROUND: A venous lake lesion is a venous ectasia that occurs on the exposed skin of elderly people. Although a number of therapies such as surgical excision, laser therapy, infrared coagulation, cryotherapy and sclerotherapy have been used to treat venous lakes, there is no guideline for treating this lesion. OBJECTIVE: The purpose of this study was to determine whether 0.5% sodium tetradecyl sulfate (STS) is effective for the treatment of venous lake lesions. METHODS: Twelve patients with venous lake lesions were enrolled In this study. After proper antiseptic preparation, 0.5% STS was slowly injected into each subject's lesion, and this was followed by immediate compression for 10 minutes. RESULTS: After treatment, all of the patients' lesions cleared completely. The average number of treatments was 2.15+/-1.28. Two patients experienced mild side effects such as light pain and paresthesia, and these soon disappeared. There were no serious side effects reported during treatment. The mean follow up period was 29.58+/-13.48 months. CONCLUSION: We have demonstrated that sclerotherapy with 0.5% STS was quite effective for treating venous lake lesions, and this treatment caused no serious adverse effects.

RESUMO

Pilar sheath acanthoma is a rare, benign follicular hamartoma commonly affecting middle-aged and elderly individuals. It is characterized by a small, solitary, skin-colored papule situated on the head and neck, particularly around the upper lip. The lesion shares some features with trichofolliculoma and dilated pore of Winer, so it should be differentiated from these. Cytokeratin (CK) is a useful marker for studying the origin of epithelial tumors. We report a case of pilar sheath acanthoma which originated from outer root sheath using CK.

Milia-like idiopathic calcinosis cutis is a rare entity. It is characterized by micronodular, whitish papules resembling milia. Histopathological examination shows small nodular foci of calcium within the superficial dermis, which are surrounded by thick collagen and inflammatory cells. Although the pathogenesis and treatment remain unclear, spontaneous resolution of the lesions may occur. We report a case of milia-like idiopathic calcinosis cutis in a 7-year-old girl who improved after treatment with tretinoin cream.

RESUMO

Clear cell sarcoma is a rare malignant tumor representing about 1% of soft tissue tumors. It usually presents in the distal extremities of young adults, frequently attached to tendons or aponeuroses. This slowly progressive tumor tends to recur and results in eventual development of metastatic growth. Early recognition of the disease and prompt wide excision of tumor are essential to get a favorable outcome. We report a rare case of clear cell sarcoma in a 57 year-old female who presented with an erythematous hard nodule on her abdomen.

RESUMO

Cutaneous, diffuse, large B-cell lymphoma (DLBCL) is characterized by a diffuse dermal infiltration which is composed of large centroblasts and immunoblasts. It is subdivided into a primary form arising from the skin and a secondary form which is metastasized from the lymph node. Secondary DLBCL has a more aggressive clinical course than the primary form. We report a 39-year-old female patient who presented with a 2 cm sized, erythematous, subcutaneous nodule on her left upper arm. She had been treated for brain-diffuse, large B-cell lymphoma which occurred 5 years before. The histopathologic and immunohistochemical examinations of the skin lesion also showed diffuse, large, B-cell lymphoma.

RESUMO

Extraskeletal chondroma is a relatively-uncommon, benign, cartilagenous tumor of the soft tissue unrelated to bone. It occurs most frequently in the hands and feet of adults in the fourth and fifth decades and presents as a solitary, painless, slowly-growing mass usually measuring <3 cm in diameter. Histologically, it can resemble hyaline cartilage or chondrosarcoma of bone, which is more cellular. Whatever the histologic appearance is, the behavior is uniformly-benign and it does not metastasize so the recommended treatment is local excision. We report a rare case of extraskeletal chondromas in a 30 year-old female who presented with a skin-colored, hard and movable nodule on her right cheek.

RESUMO

Papular elastorrhexis is a rare disorder in which children or adolescents present with asymptomatic, non-follicular, 1~5 mm firm white papules on the trunk and extremities. Histologically, there are focal areas of collagen homogenization with decreased and fragmented elastic fibers. Most reported cases are sporadic, but familial occurrence has been described. We report an uncommon and interesting case of papular elastorrhexis in both a brother and sister.

RESUMO

Bier's spots are small, irregular, hypopigmented macules that are usually found on the arms and legs of young adults. These spots are associated with venous stasis and usually appear in physiological conditions. The hypopigmented macules can be elicited by placing an affected limb in the dependent position, and disappear when the limb is raised. We report a rare case of Bier's spots associated with polycythemia vera on both forearms in a 26-year-old male. We think the hyperviscosity induced by polycythemia vera might be part of the pathogenesis of this phenomenon.

RESUMO

Nevus lipomatosus cutaneous superficialis (NLCS) is a fairly uncommon lesion characterized by ectopic adipose tissue in the dermis. It presents as soft yellowish papules or cerebriform plaques occurring usually on the buttock or thigh with a wrinkled surface. We report a case of NLCS in 38-year-old female who presented reddish brown colored cerebriform tumors on her buttock and was treated by CO2 Laser. To our knowledge this is the first case of NLCS treated by CO2 Laser, path of except by excisional surgery.

RESUMO

Estrogen dermatitis is a rare cutaneous disorder characterized by recurrent cyclic eruptions during the luteal phase of the menstrual cycle. The clinical manifestations include pruritus, urticaria, vesicles, papules and eczematous eruptions. A 22-year-old female patient presented with a skin lesion on the back of 6 years periodic duration. She had been recurrently suffering from pruritic eruptions that appear during the luteal phase of the menstrual cycle and resolve after menses. An intradermal skin test to estrogen showed a positive result, but showed negative results to progesterone. After antiestrogen tamoxifen therapy, the lesions were found to have significantly improved.

BACKGROUND: Quality of life (QOL) is a broad concept that encompasses all aspects of an individual's existence. Acne is a common inflammatory dermatosis which most frequently affects the face, and produces significant psychological and physical scarring. OBJECTIVE: The aim of this study was to compare QOL in patients with acne and healthy controls by using the WHO QOL Scale, and to assess QOL of patients with acne by using the generic (WHO Quality of life scale), dermatology-specific (Skindex-29) and acne-specific (APSEA) questionnaires. The relationship between clinical characteristics and QOL were explored. Additionally, financial values which patients attach to the treatment for their acne were determined. METHODS: 170 acne patients and 41 healthy controls were recruited in this study and completed the WHO QOL- BREF Scale. Total scores and scores of domains of the WHO QOL Scale in the two groups were compared. Correlation analysis and multiple regression analysis were performed to examine the relationship between the quality of life and the clinical characteristics of patients with acne. RESULTS: Total score of WHO QOL scale in patients with acne were decreased compared to those in healthy controls. Clinical characteristics, including the severity of depression and anxiety, disease duration, clinical severity and self-reported severity (SRS), showed significant correlation with QOL. Multiple regression analysis revealed that the severity of depression and anxiety, and SRS were factors directly affecting the QOL in the patients with acne. As for financial aspects, 27.6% of patients would pay 100,000~500,000 won for acne treatment, but 33.5% of patients answered they would not pay when cost of treatment exceeded 100,000 won. CONCLUSION: QOL in patients with acne was poorer than that of healthy controls. The association between QOL and SRS was stronger than QOL and clinical severity. The severity of depression and anxiety was the most important predictor on QOL.
